ORDER TRANSCRIPTS

Students requesting SCI-Arc transcripts should complete this PDF form and return it to the registrar or academic counselor with the appropriate fee. Transcript copies are $5. (This fee is waived for currently enrolled students).

Students may also send written, signed requests via letter or fax to:

Students should include the following information with their request:

1. Students' full name
2. Student ID number (if applicable)
3. Current mailing address
4. Student's signature
5. Complete name and address of the institution/individual where transcripts are to be sent.
6. Check/Money Order/Visa or MasterCard information for the appropriate amount. If using a credit card, please do not send this form electronically
